For each line item below, fill in a check ☑ for pass, and a cross ☒ for failed. All failed items must include photos and detailed notes of each failed item (example: missing GFCI outlet in the upstairs bathroom, or failing outlet missing covers in first floor bedroom, living room, and kitchen), along with location of each incident (Kitchen, Bathroom, Bedroom 1, Bedroom 2, Basement, Laundry Room, Exterior, Parking Lot, Roof, Porch, etc). General Item 1 – A working smoke detector and carbon monoxide detector with a live battery must be installed on every level of the unit, including the basement and outside of sleeping rooms. This prevents exposure to possible lead-based paint hazards.
