Work Order# 256378 Property Address: 10724 Gooding Ave DN Unit Cleveland, OH 44108 Description of Findings: Upon inspection, we found the kitchen needs significant remodeling. The drop ceiling is wet and has organic growth. The kitchen perimeter wall has a wallpaper-like texture on top of the drywall. After checking the base of the cabinet, we found the drywall area is all wet and has organic growth. To address this properly, we have to remove all these wet materials and go up to the studs. If any possible plumbing damage is found after removal, we will call from the site. From the water damage, the kitchen sink cabinet is rotten. We have to rebuild it using water-resistant materials. All the windows need screens. Organic growth treatment is not included in the estimate as we have to check the current stud condition. The treatment and replacement must be done after the source is addressed. The inspection was completed based on the areas the tenant pointed out. If any additional damage is found while working, the client will be notified from the site. Thank you. Estimates: SN 1. 2. Description of Service/Materials Kitchen ceiling removal: - Disconnect and remove 100 SF of ceiling tiles from the kitchen. Sink cabinet removal: - Shout off the water line to the sink. - Dismantle and remove the drain and supply lines. - Remove the sink base and faucet. - Dismantle the cabinet countertop and store it aside. - Dismantle and remove the rotted cabinet. Haul away and dump all the hazardous debris generated from the project. Plumbing Diagnostic: After the area is open, perform a plumbing diagnostic to determine the source of leaks and how to fix them. Organic Growth Inspection: Perform an organic growth inspection and the current stud condition to determine the possible treatment for the hazardous condition. Kitchen ceiling replacement: - Replace 100 SF of ceiling tiles inside the kitchen and connect with the existing T-frame. **Work must be performed after the plumbing and organic growth is addressed.** Ceiling materials: White drop ceiling 100 SF.
