Re: Ong~ing Monitoring and Maintenance Plan Requirement
Dear Lofty Holding 9902:
Please find enclosed an Ongoing Monitoring and Maintenance Plan Acknowledgement form for
the property located at 9902 Garfield Ave, Cleveland, OH 44108-2122. Because interim control
methods were utilized to control the lead hazards listed below in the above stated residence,
you must implement an ongoing monitoring and maintenance plan in accordance with 3701-3010 of the Ohio Administrative Code (O.A.C.).
Ongoing monitoring and maintenance requires the following:
1. The property owner conduct an annual visual survey of any remaining lead based

paint that was controlled utilizing interim control methods.
2. The property owner have a licensed clearance technician, lead inspector or lead
risk assessor to conduct a clearance reevaluation which involves dust sampling and/or
soil .sampling to determine if lead hazards exist.
As acknowledgement of the ongoing monitoring and maintenance requirements, the enclosed
form must be signed by you and mailed or faxed to the following:

Failure to complete the ~nclosed form may prompt the department to issue a noncompliance
notice and order to vacate pursuant to rule 3701-30-11 of the O.A.C.
