Code Section 1311.06 State of ___________________ ) ) SS: County of ___________________ ) Nathan Fisher , whose address is 18318 Pearl Road , Strongsville, Ohio 44136 , being first duly sworn, says that Aligned Properties , the lien claimant, furnished certain material or performed certain labor or work in furtherance of improvements located on or removed to the land hereinafter described, in pursuance of a certain contract, with Calixte Duffaut DBA Yhome Nursing L LC, whose address is 52 Point Pleasant Drive , Palm Coast, FL 32164 , on behalf of Lofty Holding 3024 W . 103rd Street D AO LLC , the owner, part-owner, lessee, original contractor, subcontractor, or other person, as the case may be, whose address is 66 W Flagler St., Num 900, Miami, FL 33130 (See Ex. 1 attached herein). The contract was updated once they switch ed the landlord from Calixte Duf f aut to ECO Systems LLC c/o Earl Vanze Co on behalf of Lofty Holding 3024 W . 103rd Street D AO LLC, the owner, part-owner, lessee, original contractor, subcontractor, or other person, as the case may be (See Ex. 2 attached herein). The first of the labor or work was performed or material was furnished on the 19th day of November, 2024.
