If applicable, a list of the required repairs and responsibilities is listed below. A re-inspection has been scheduled for 10/31/2022 between the hours of 12:00 PM - 5:00 PM. Owner: Failure to correct all violations listed on the attached page(s), which are deemed owner responsibility in accordance with HQS will result in termination of your HAP contract. Abated monies cannot and will not be repaid to you. Tenant: As a recipient of housing subsidy, you are required to allow CMHA to inspect your unit in order to remain eligible for housing assistance. If you are not present at the specified inspection time and/or fail to have someone over the age of 18 present for the inspection, your subsidy may be terminated. If your unit has deficiencies which are deemed tenant responsibility, it is your responsibility to correct the deficiencies by the re-inspection date. If the deficiencies are not corrected, or unit is not made available for inspection, you may be terminated from the Housing Choice Voucher Program. NOTICE: CMHA, reserves the right to cite additional HQS violations upon re-inspection of the unit, should conditions at the time of re-inspection warrant this.
