Ashdod City Corp., whose address is 4161 Stonehaven South Euclid, Ohio 44121, being first
duly sworn, says that the lien claimant furnished certain material or performed certain labor or
work in furtherance of improvements located on or removed to the land hereinafter described,
pursuant to a certain contract, with the owner, partowner, lessee, original contractor,
subcontractor, or other person, as the case may be, whose name is Yellowstone Lofty Holding
13806 Coit Rd DAO LLC, a Wyoming Decentralized Autonomous Organization Limited
Liability Company and whose address is 13806 Coit Rd. Cleveland, OH 44110.
The labor, work, or materials provided by the claimant consisted generally of: Interior and
exterior improvements including flooring installation, cabinet and countertop painting, lighting
installation, window and door repairs, bathroom renovations, baseboard repairs, exterior
painting, porch siding repairs, HVAC installation, plumbing work, smoke and CO2 detector
installation, steps repair, trash removal, and related labor and materials.

The first of the labor or work was performed or material was furnished on the 1 st day of April
2025. The last of the labor or work was performed or material was furnished on the 10st day of
August 2025, and there is justly and truly due the lien claimant therefore from the owner,
partowner, lessee, original contractor, subcontractor, or other person, whose name is
Yellowstone Lofty Holding 13806 Coit Rd DAO LLC, as the case may be, the sum of
$32,460.00, after allowing all credits and setoffs, for the labor performed and/or materials
furnished, for which amount, the lien claimant, claims a lien on the land, building or leasehold,
of which Yellowstone Lofty Holding 13806 Coit Rd DAO LLC is or was the owner, partowncr,
or lessee, as the case may be, which premises are described as follows:

And known as being Sublot No. 12 in Glenhaven Subdivision of part of Original
One Hundred Acre Lot No. 359 as shown by the recorded plat in Volume 38 of
Maps, Page 8 of Cuyahoga County Records, and being 40 feet front on the
Southwesterly side of Coit Road N.E., and extending back of equal width 120
feet, as appears by said plat, be the same more or less, but subject to all legal
highways.
By virtue of the Ohio mechanic's lien laws (O.R.C. Chapter 1311), the claimant asserts a lien
upon the above-described premises for the amount due.
